Telehealth Primary Care in New Jersey
Telehealth primary care in New Jersey means seeing the same board-certified family doctor by secure video, wherever you are in the state. Dr. Nelly Singh, DO offers online direct primary care to anyone physically located in NJ — unhurried 30 to 60 minute visits, no copays, and no insurance required. It's real, relationship-based family medicine, delivered through your phone or laptop.
Online direct primary care, for the whole state.
Direct primary care (DPC) is a membership model that works directly between you and your physician, outside of the insurance system. Delivered by telehealth, it gives you a virtual family doctor in New Jersey who actually has time for you — no rushed seven-minute appointments, no rotating providers, and no surprise bills.
Because visits happen over secure, HIPAA-compliant video, this care is available to anyone physically in New Jersey at the time of the visit, from Bergen and Sussex down to the Shore. You don't need to live near an office, take time off to sit in a waiting room, or fight traffic. You just need a private spot and an internet connection somewhere inside the state.
Dr. Nelly does not bill insurance, so there are no copays and no claims. A superbill is available on request if you'd like to seek out-of-network reimbursement, and HSA/FSA funds may be applied toward membership.
Three simple steps.
From first visit to ongoing care, telehealth keeps things straightforward.

What still needs to be in person.
Telehealth handles a great deal, but not everything. Some things genuinely require hands-on care, and Dr. Nelly will tell you plainly when that's the case. A few examples that aren't done over video include physical exams that require in-person evaluation, pap smears, immunizations, biopsies, joint injections, and laceration repair. Controlled substances are also not prescribed through telehealth.
When you need one of these, Dr. Nelly either refers you to the right place or, in select North Jersey communities, handles it during an in-person home visit. A physical office is opening soon in Waldwick, NJ as well. The goal is simple: get you the right care, in the right setting, without sending you on a runaround.
A great fit if you want a doctor who's actually reachable.
Busy professionals & parents
Care that fits around work and family, without a half-day spent in a waiting room. Visit from home, the office, or anywhere in NJ.
People without convenient access
If finding a primary doctor with timely openings has been a struggle, statewide telehealth gives you direct access from wherever you are.
Anyone tired of insurance hassles
Flat, transparent pricing with no copays and no surprise bills. Keep your insurance for hospitals, imaging, and specialists.
People managing chronic conditions
Steady, ongoing follow-up with the same physician who knows your history, plus direct messaging between visits.

Telehealth primary care questions.
Is telehealth primary care legit and effective?
Yes. Telehealth primary care is delivered by Dr. Nelly Singh, DO, a board-certified family medicine physician, over secure HIPAA-compliant video. Most everyday primary care — chronic condition management, acute illness, mental health, prescriptions, lab orders, and referrals — is handled well by video. When something genuinely needs hands-on care, Dr. Nelly tells you plainly and either refers you out or handles it during a home visit in select NJ communities.
Do I need to be in New Jersey for a telehealth visit?
Yes. You must be physically located in New Jersey at the time of your visit. Telehealth is available statewide to anyone in NJ — you don't need to live near an office. You just need a private spot and an internet connection somewhere inside the state when your appointment begins.
Can Dr. Nelly prescribe medications online?
Yes, when it's medically appropriate. Dr. Nelly can send e-prescriptions and refills to your pharmacy and order labs directly. Because visits are virtual, controlled substances are not prescribed through telehealth. If a medication or service requires in-person evaluation, she'll guide you to the right setting.
Does telehealth primary care work without insurance?
Yes. This is a cash-pay direct primary care model, so no insurance is needed and there are no copays or surprise bills. It's $90/month for the Telehealth Membership or $120 for a single visit. HSA/FSA funds may be applied toward membership, and a superbill is available on request for possible out-of-network reimbursement. Keep your insurance for hospitals, imaging, and specialists.
How fast can I be seen?
Members can often be seen the same day or next day. Pay-per-visit patients can book when appointments are available, typically within a few days. Members also get direct messaging with Dr. Nelly between visits, so quick questions don't always require a full appointment.
