Signature Care
$140/mo adults · $90/mo ages 5–25
All visits included, in the office or by video. Same-day or next-day appointments, 30 to 60 minute visits, and direct messaging with Dr. Nelly.
Telehealth
$120 per visit
Video visits anywhere in New Jersey, up to 20 minutes. For simple visits and mental health follow-ups. Not for long-term medications or anything that needs a physical exam.
Single office visit
$200 per visit
One in-person visit with Dr. Nelly. No membership.
Osteopathic treatment (OMT)
$250 first visit · $160 follow-up
Hands-on care for back and neck pain, headaches, and joint stiffness. Saturday and Sunday, 11am - 4pm. No membership required.

Do you make house calls?
Home visits are part of Concierge Care, a higher level of care with limited enrollment and a waitlist. Signature Care members are seen at the Waldwick office and by telehealth.
How much does membership cost?
Signature Care is $140/month for adults 26+ and $90/month for ages 5 to 25, with all visits included. It starts with a free trial, so you are not charged until your first visit.
Do I need insurance to join?
Insurance is not required and is never billed. Your membership covers all visits. You can still use insurance for labs, imaging, specialists, and medications. A superbill is available on request, and membership is HSA eligible.
