What is direct primary care?
A simpler, more personal way to access your doctor.
In many traditional healthcare settings, visits are often brief and time-constrained — sometimes only a few minutes of face-to-face time, as noted in The New York Times.
With a membership, you can connect directly with your physician — without many of the usual barriers of traditional healthcare.

Do you accept insurance?
We don't bill insurance for visits.
This allows for:
- more time
- better access
- care focused on you — not insurance requirements
You can still use insurance for labs, imaging, specialists, and medications. A superbill is available upon request to submit to your insurance for possible reimbursement. Reimbursement depends on your specific plan and out-of-network benefits.
I have good insurance. Do I still need a membership?
Many of our patients have excellent health insurance.
Membership gives you:
- timely appointments
- direct communication
- continuity with your doctor
Insurance covers costs. Membership improves your care.
Do I still need insurance?
Yes. Health insurance is recommended for hospitalizations, specialists, high-cost imaging or procedures, and unexpected medical events.
Can I join if I have Medicare?
At this time, we are not able to enroll patients with Medicare.
You are welcome to join our Medicare waitlist, and we will reach out when this changes.
Is this concierge medicine?
It shares similarities — increased access, longer visits, and house visits — but direct primary care is designed to be more transparent and accessible.
A higher-touch Concierge Direct Care option is also available for patients in Central and North Jersey, which includes in-person and house visits and priority access. A complimentary meet & greet with Dr. Nelly is required to evaluate fit.
Can I use HSA or FSA funds?
Yes. Health Savings Account (HSA) funds may be used toward direct primary care memberships, up to monthly limits defined by federal guidelines.

How quickly can I be seen?
Members can often be seen the same day or next day. Pay-per-visit patients can book when available — typically within a few days.
Will I see the same doctor?
Yes. Care is built around an ongoing relationship with your physician — not rotating providers.
Where do I need to be for my visit?
Telehealth is available to any patient located in New Jersey at the time of their visit.
In-person and house visits are available for Signature Care members in Central and North Jersey.
Do you prescribe medications?
Yes — when medically appropriate.
Due to the virtual nature of visits, controlled substances are not prescribed at this time.
Can I cancel my membership?
Yes. You can cancel at any time.
Are visits virtual or in person?
Telehealth is available to anyone in New Jersey.
In-person and house visits are available for Signature Care members in Central and North Jersey.
