What is concierge-style direct care?

A simpler, more personal way to access your doctor.

In many traditional healthcare settings, visits are often brief and time-constrained, sometimes only a few minutes of face-to-face time, as noted in The New York Times.

With a membership, you can connect directly with your physician, without many of the usual barriers of traditional healthcare. Learn more about direct primary care in New Jersey.

How do I become a patient?

There are four ways to start with Dr. Nelly:

  • Pay per visit — book a single telehealth visit on the scheduling page, no membership required.
  • Telehealth Membership — sign up and book your first visit in the patient portal.
  • Signature Care — sign up in the patient portal for in-office and telehealth care at the Waldwick, NJ office.
  • Concierge Care — start with a complimentary meet & greet so Dr. Nelly can confirm your location is in her service area before enrolling.
Do you accept insurance?

We don't bill insurance for visits.

Your monthly membership fee covers your visits with Dr. Nelly.

This allows for:

  • more time
  • better access
  • care focused on you, not insurance requirements

You can still use insurance for labs, imaging, specialists, and medications. A superbill is available upon request to submit to your insurance for possible reimbursement. Reimbursement depends on your specific plan and out-of-network benefits.

I have good insurance. Do I still need a membership?

Many of our patients have excellent health insurance.

Membership gives you:

  • timely appointments
  • direct communication
  • continuity with your doctor

Insurance covers costs. Membership improves your care.

Can family members join?

Yes. Concierge Care includes a Household plan that covers 2 adults and can include up to 2 dependents, all at one flat monthly rate.

For the Telehealth and Signature Care memberships, each adult enrolls individually.

Is this concierge medicine?

It shares similarities, increased access, longer visits, and home visits, but direct primary care is designed to be more transparent and accessible.

You pay one flat monthly membership. Unlike in traditional concierge medicine practices, there is no insurance billing for appointments, retainer fees, co-pays or surprise costs. Just your healthcare between you and your doctor.

Signature Care covers in-office and telehealth visits at the Waldwick, NJ office. For patients in select Central and North Jersey communities, Concierge Care adds in-person home visits and after-hours access on top of in-office and telehealth care. A complimentary meet & greet with Dr. Nelly is required to enroll in Concierge Care.

Do I still need insurance?

Yes. Health insurance is recommended for hospitalizations, specialists, high-cost imaging or procedures, and unexpected medical events.

Can I join if I have Medicare?

At this time, we are not able to enroll patients with Medicare.

You are welcome to join our Medicare waitlist, and we will reach out when this changes.

Can I use HSA or FSA funds?

Yes. Health Savings Account (HSA) funds may be used toward direct primary care memberships, up to monthly limits defined by federal guidelines.

Please check with your plan for details.

How quickly can I be seen?

Signature Care and Concierge Care members can often be seen the same day or next day, in person at the Waldwick, NJ office or by telehealth.

Telehealth Membership patients can typically book a video visit within a day. Pay-per-visit patients can book when available, typically within a few days.

How often will I see Dr. Nelly?

As often as your care needs.

  • Telehealth Membership includes 2 visits per month with next-day scheduling.
  • Signature Care adds same-day or next-day in-office visits at the Waldwick, NJ office.
  • Concierge Care covers in-office, telehealth, and home visits for sick care, wellness, and your annual physical.
  • Pay per visit patients book one telehealth visit at a time, no membership required.
Will I see the same doctor?

Yes. Care is built around an ongoing relationship with your physician, not rotating providers.

Where do I need to be for my visit?

Telehealth is available to any patient located in New Jersey at the time of their visit.

Home visits are available for Concierge Care members in select North and Central Jersey communities. The Waldwick, NJ office opens July 13 for in-office Signature Care and Concierge Care members.

Do you prescribe medications?

Yes, when medically appropriate.

Controlled substances are not prescribed for telehealth visits.

How will my medical records be handled?

Your records are kept in a secure, HIPAA-compliant electronic health record. Because Dr. Nelly is the only physician documenting in your chart, your history stays continuous from visit to visit.

You can view records, complete intake forms, schedule visits, and message Dr. Nelly directly through the patient portal.

Can I cancel my membership?

Yes. You can cancel at any time.

Are visits virtual or in person?

Telehealth is available to anyone in New Jersey.

Home visits are available for Concierge Care members in select North and Central Jersey communities. The Waldwick, NJ office opens July 13 for in-office Signature Care and Concierge Care members.
